Pharmacist
• Runs all daily and regular reports required from both Medics and automated dispensing machine.
Uses Microsoft Word and Excel to prepare basic documents and analysis.
• Explains and demonstrates the various Medics functions.
Scrutinizes a profile to determine what medication regimen was entered and if it agrees to the order. Enters medication orders in Medics.
• Prints labels or charge for medications to be dispensed to nurses.
• Prints labels for unit dose packaging of solids and liquids (as applicable).
• Compounds, dispenses, and restocks Medication.
Obtains census from admissions and prints one from MEDICS.
Compares, adjusts and checks for name alerts.
Restocks emergency night IV med cart and checks for new orders.
Prints restock or cart fill lists and fills medications for all nursing units.
Goes to floors and restocks automated dispensing machines or narcotic cabinets.
Records refrigerator temperature on the logs.
Puts away Cardinal order and compares to invoice. Note Discrepancies.
Checks for outdated medications according to the schedule and labels the bins outdating soon.
Demonstrates ability to repackage medications by printing labels and packages liquid or solids into unit dose format.
Orders Entry and Problem Resolution.
Cleans and documents cleaning of IV hood.
• Runs the end of day charges and send the hard copy to the business office.
• Prepares IV's in compliance with state laws and uses aseptic techniques. Prints IV's Intravenous and Intermittent.
• Demonstrates the ability to counsel patients about their medications.
• Uses common reference sources to identify medications, drug interactions, or recommended regimens. Modifies a drug regimen to take into account patient's renal function.
Responds appropriately to a suspected drug allergy warning on when encountered on MEDICS.
Identifies orders not to be entered because of problems in regimen.
Demonstrates the proper procedure towards resolving problems in regimen.
Demonstrates knowledge of common drug interactions.
Provides an appropriate response to a drug information question.
Completes a patient medication reconciliation, and knows how to resolve problems.
• Orders medications.
If part of your job, prepares and submits a purchase order to the wholesaler.
If part of your job, reconciles invoices to end of month wholesaler statement using manual or AccountSource Rx system.
• Restocks narcotics, and delivers and properly enters narcotics to the floor.
Explains the process for identifying drugs that need to be restocked.
Removes narcotics that have expired from the floor units and assign as waste.
• Receives narcotics and properly completes DEA Form 222.
Enters narcotics into perpetual inventory and narcotic vault.
• Demonstrates familiarity with the drugs that are formulary and non-formulary.
Provides the names of cost-effective formulary medications when asked for a recommendation by a physician. Uses the substitution list when a non-formulary product is ordered.
• Handles all pharmaceutical generated protected health information consistent with HIPPAA.
• Calculates appropriate dose and interval for drugs requiring pharmacokinetic monitoring.

• Describes the action to take when a warning is generated using the pharmacy profile system.
• Describes normal lab values for commonly encountered labs.
• Complies with the National Controlled Substance Policy and diversion prevention procedures.
Prepares new policies as required and ensures the policy manual is kept current
• Reviews profile overrides daily and compares to pharmacy orders.
Documents and keeps track of interventions with physicians.
Reviews and follows up on computer generated discrepancies from automated dispensing machines.
• Complies with Joint Commission Patient Safety Goal regarding patients receiving anticoagulation.
• Organizes, plans, and manages time effectively to complete assignments.
• Meets position requirements and performs essential functions.
• Completes mandatory training and courses required by completion date.
• Reports questionable situations, concerns, complaints or harassment immediately.

Credentials:
• Licensed in good standing by the State Pharmacy Board. For the State in which the hospital is located.
Minimum Qualifications:
• Successful completion of a degree from an accredited pharmacy program or one that is determined to be substantially equivalent to a U.S. accredited program by an appropriatecertifying agency preferred.
• 1 year of supervisory experience preferred.
Machines, Equipment Used:
• General office equipment such as computer/laptop, telephone, copy/fax machine, calculator, scanner, etc.
• Microsoft Office software, to include Outlook, Word, and Excel.
Physical Requirements:
• Visual acuity, speech recognition, speech clarity.
• Ability to lift, lower, push, pull, and retrieve objects weighing a minimum of 30 pounds including medical supplies and equipment and the transferring and repositioning of patients. Reasonable assistance may be requested when lifting, pushing, and/or pulling are undertaken which exceeds these minimum requirements.
• Ability to withstand prolonged standing and walking.
• Ability to reach, stoop, bend, kneel, and crouch are required for patient care functions and in setting up and monitoring
equipment.
Skills and Abilities:
• Oral communication, written communication, active listening. Must be able to speak and understand English.
• Information ordering, deductive reasoning, social perceptiveness, time management, critical thinking.
• Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner.
• Ability to work independently without continuous supervision.

• Indoor, temperature controlled, smoke-free environment.
• Handicapped accessible.
• May work under stressful circumstances at times.
Proficiency or Productivity Standards:
• Has regular, reliable, and predictable attendance and punctuality.
• Adheres to dress code including wearing ID badge.
• Adheres to Standards of Business Conduct.
• May be required to work weekdays and/or weekends, evenings and/or night shifts if needed to meet deadlines.
• May be required to work on religious and/or legal holidays on scheduled days/shifts.
• May be required to perform other duties as assigned by supervisor.
